Criminal Record Checks

 Criminal Records Checks
To help provide a safe environment for our players, the Midnapore Hockey Association requires all Coaches and Assistant Coaches
to be screened via the Calgary Police Service. All of these volunteers will be required to complete a Criminal Records Check.
*If you completed your Criminal Records Check last season, it is on your file and you do not need to submit again this year.
*If you do not have a verified Criminal Records Check on file with Midnapore Hockey, please come prepared to complete this form
during the upcoming Coach meetings. Proper identification is required. Bring a high quality photocopy of a picture ID.
Examples are your Driver’s License or Passport. As well as a copy of another government issued piece of ID
such as your Alberta Health Care Card, Birth Certificate, or Social Insurance Card.
